Different riboflavin formulations affect central corneal thickness during cross-linking

Isotonic riboflavin with dextran used during cross-linking was found to significantly decrease corneal thickness, while dextran-free isotonic riboflavin significantly increased corneal thickness.

A retrospective study included two groups of patients with progressive keratoconus. Group A consisted of 100 cases that underwent saturation with isotonic riboflavinand dextran, and group B consisted of 100 cases that underwent saturation with isotonic riboflavin with hydroxypropyl methylcellulose.

Group A cases experienced a mean decrease of 51.4 µm in central corneal thickness (CCT). Group B cases experienced a mean increase of 109.4 µm in CCT. After ultraviolet A irradiation, group A cases experienced a mean decrease of 46.7 µm in CCT and group B cases experienced a mean increase of 59.2 µm in CCT.

“Various formulations of riboflavin have different effects on corneal thickness during cross-linking. Riboflavin with dextran causes a significant intraoperative decrease in corneal thickness, and riboflavin with [hydroxypropyl methylcellulose] causes a steady, maintained and significant increase in corneal thickness,” the study authors wrote. – by Robert Linnehan
